The North Carolina Vaccines for Adolescents Program
The main objective for the North Carolina Vaccines for Adolescents (NCVAP) is to immunize every adolescent ages 11-12 for Tdap, meningococcal disease and young females ages 11-12 for HPV within the state of North Carolina.

NCVAP will provide clerical staff to assist primary care and pediatric practices recall adolescent patients for CDC recommended vaccines. The NCVAP program offers assistance, free of charge, with patient data input for primary care and pediatric practices that currently use the NCIR system. The required data elements are:

1. Patient name, address, parents or guardian's name and date of birth.
2. The patient birthdates should be from April 1, 1997 thru March 31, 2000.

This approach will allow primary care pediatric physicians to perform the recall function and send reminder letters to the parents for age appropriate (11-12 year old) adolescents for recommended vaccinations.
