CapitalCare Collaborative

The CapitalCare Collaborative (CCC) is a membership of safety net providers working collaboratively to develop initiatives to improve the health of the region's medically underserved. 

The CCC, a program of the Wake County Medical Society, was formed in April 2006 to provide an unprecedented coordination of care in Wake County.  The CCC will utilize centralized, web-based tools to assist local health care safety net providers in more efficiently providing services, thus having a positive impact on a provider’s capacity to serve the uninsured with high quality medical care.

The CCC consists of eight healthcare providers in Wake County who provide the bulk of medical care to the county’s uninsured and underinsured population.

Urban Ministries of Wake Co. Open Door Clinic
Wake County Human Services
Wake County Medical Society
Wake Health Services, Inc.
