The Community Care Program of Wake and Johnston Counties (CCWJ) is one of 16 community networks under Community Care of North Carolina. CCWJ has created private and public partnerships to improve the management and delivery of care for Carolina ACCESS Medicaid recipients. Our goal is to build a community-based system that will achieve access, quality, utilization and cost objectives. CCWJ has put management tools and processes in place to achieve improved performance with disease management initiatives such as asthma, diabetes, emergency department and pharmacy utilization. CCWJ has a case management team that supports local physicians regarding best practices and intensive patient follow-up to assure timely access to high quality care.
