General Information
1. Eligibility -Project Access is determined by total family income, proof of county
residence, and insurance status. Eligibility screening is based on physician referrals only.
2. Enrollment -Patients are enrolled
in Project Access by an enrollment site representative after screening has been completed. Once approved, the patient will
receive a Project Access ID card which must be presented at all appointments.
3. Referral Process -Enrollment site physicians will determine whether a specialty referral is required for the patient. Compliant patients are the best candidates for referrals since Project Access is entirely a volunteer program.
4. Appointment
Process -Appointments
are scheduled by the Project Access community health worker. The
patient will be identified to the referring physician's office as a Project
Access patient.
