Project Access Patient Information


If you need health care and are uninsured, unemployed or a low wage earner, call to schedule a "New Patient" appointment at one of the clinics listed below.

The key to maintaining good health is to see your primary healthcare provider on a regular basis. Your doctor at one of the clinics will evaluate whether a referral to a PA specialist is necessary.

Project Access is a physician referral program. Patients cannot apply directly to the Project Access office for services, you must be screened by one of the clinics listed.

General Information


1. Eligibility - Project Access is determined by total family income, proof of county residence, and insurance status.  Eligibility screening is based on physician referrals only.

2. Enrollment - Patients are enrolled in Project Access by an enrollment site representative after screening has been completed. Once approved, the patient will receive a Project Access ID card which must be presented at all appointments. 

3. Referral Process - Enrollment site physicians will determine whether a specialty referral is required for the patient.  Compliant patients are the best candidates for referrals since Project Access is entirely a volunteer program.

4. Appointment Process - Appointments are scheduled by the Project Access community health worker. The patient will be identified to the referring physician's office as a Project Access patient.
