Project
Access helps Wake County physicians demonstrate their commitment
to the ideals of their profession in an organized efficient and
rewarding way that is properly recognized and clearly defined.
Organized Way
Project Access identifies eligible patients for referral to your
practice in order to help you meet the level of commitment you
choose.
For example, Project Access employs a patient reminder system
and coordinates the flow of information between the enrollment
site (referral clinic) and private practices. Project Access works
closely with your office staff to make sure that the uninsured,
low-income patients you see use your services properly.
An Efficient Way
Project Access ensures that the time your physicians devote to
their patient is used to maximum benefit
and that your skills are applied where and when they are most
needed and appropriate. Project Access also ensures the equitable
expenditure of your effort to help the uninsured, low-income
people of our community.
Project Access will
- Help you define your level
of commitment;
- Refer patients only as appropriate to your
practice;
- Help your patient acquire needed medications;
- Provide current information for further referrals which
may be required;
- Provide hospitalization, if needed; and
- Provide
reports to you and your staff as requested.
A Properly Recognized Way
Project Access will
foster greater public recognition and appreciation for the Wake
County medical community’s
role in serving the uninsured-low-income patients. We provide
a strong program
to increase media awareness and recognition of physician volunteerism;
By recognizing your volunteer work on an ongoing
basis, Project Access helps the people of Wake County understand
the community’s
commitment to improving access to needed health care.
Enrolling Your Own Patient(s)
If you are a Specialist
and have a patient who may be eligible for Project Access, complete
the Physician
Acceptance Form and
fax to the PA office. Staff will screen for eligibility and
enroll the patient into PA, pending approval.
Join the Network of PA Providers - If you wish to join the network of PA providers and pledge care for any number of patients per year, download this pledge form and fax to Pam Carpenter - Physician Pledge Form at 510-9162.
Physician Office Staff
1. Appointments—A Project Access community health worker
from one of the Enrollment Site clinics will schedule the initial
appointment with your office. This clinic will be the one that
you mail the patient’s health record back to.
2. Project Access patients must show a current Project Access
ID card at each office visit. If the patient does not present
a current Project Access ID card, you may call the Project Access
office, Monday through Friday between 9 a.m. and 5 p.m. to verify
enrollment at 919-783-0404; x-20 or x-21.
3. You may consider designating a staff person in your office
for us to contact when Project Access appointments are being scheduled.
4. Additional Referrals—For additional
referrals or services (i.e. lab or diagnostic) you may contact
the Project Access office

5. Claims Processing—Project Access
patients are not to be billed for services. However, in order
to track the goodwill
care that is being contributed, practices are asked to submit
a standard HCFA 1500 claim form for services provided to:

6. Do not bill the patient for Project Access
services. To report NO SHOWS or patient noncompliance, contact
the Project Access
office at WCMS – 919-783-0404; x-21 or x-22.7. Reporting—Project
Access office tracks all commitments of physicians. It is important
to notify the Project Access office if an additional referral
needs to be made. Physicians can contact the Project Access office
to obtain a report of their patient
Eligibility Requirements
1. Patient must be a Wake County resident – U.S.
citizenship not required.
2. Patient does not have health insurance of any kind and is
not eligible for Medicare, Medicaid, CHIP or VA benefits.
3. Income – 185% of the Federal Poverty
Guideline (see chart). Must provide proof of income to Enrollment
Site Health
Care Clinic.
4. Initial enrollment is for a six-month period.
An extension may be granted, pending continued eligibility and
continued treatment
and/or medications. However, it is the patient’s responsibility
to call the Project Access Office to coordinate an extension based
on continued eligibility.
